How To Register

You can register with the practice providing you live within the practice boundary (Parts of S3, S4 and parts of S5) - please see searchable catchment area map.

If you wish to register with the practice please complete a Registration Form. Please be aware it is one form per person.  A copy of this can be downloaded here. Please return the form to the practice by post or place it in the post box on the wall outside the front door. Your form will then be sent to NHS South Yorkshire ICB who will arrange for your medical notes to be transferred from your previous doctor; this would normally take 7– 8 weeks.

You do not need proof of address, identification or an NHS number in order to register with a GP surgery. If you have these, they may help us to find your records from your previous doctor, but they are not required to register.

Our receptionists won’t ask you for proof of your immigration status.

Medical treatment is available from the date of registration. Please contact reception for more information. 

Overseas Visitors

Please note that overseas visitors are entitled to Free NHS GP services once they register with the practice. Hospital services apart from A&E may attract a charge.

Safe Surgery

All are welcome! Your nationality or immigration status does not affect your right to register here.

We are a Safe Surgery for everyone in our practice area.

• Everyone living in England has the right to free NHS care from a GP.
• Ask reception for an interpreter if you find it difficult to communicate in English.
• Our receptionists won’t ask you about your immigration status.
• If you are worried about giving us your address, please let us know. Your information is safe with us.
